Ruger - LCP Product Safety Warning and Recall Notice
DATE: October 30, 2008
Ruger has recently received a small number of reports from the field indicating that LCP pistols can discharge when dropped onto a hard surface with a round in the chamber. We are firmly committed to safety and would like to retrofit all older LCP Pistols. The retrofit involves installation of an upgraded hammer mechanism at no charge to the customer.
How to determine if your pistol needs the retrofit: All LCP pistols bearing prefix "370" (that is, serial number 370-xxxxx) may be affected. Newly manufactured and retrofitted "370" prefix LCP pistols with the new hammer mechanism installed have been marked with a diamond in the flat portion of the slot just behind and below the hammer as show to the right. Going forward, new pistols will be marked with the serial number prefix "371" (serial number 371-xxxxx). Pistols with the "371" prefix are not affected.

Smith & Wesson i-Bolt SAFETY RECALL NOTICE
Applies to i-Bolt rifles manufactured between 10/18/2007 and 12/12/2007.
DATE: April 26, 2008
Smith & Wesson has discovered that the sear engagement pin in certain i-Bolt action rifles does not meet specification. The sear engagement pin is part of the fire control unit for the rifle which is manufactured for Smith & Wesson by an outside vendor. Our investigation has revealed that some of the sear engagement pins produced for Smith & Wesson do not comply with specifications and may, in certain instances, lead to a failure of this component. Although we have not received any reports of accidents or injuries, we are initiating this recall campaign in the interest of safety since a failure of the sear engagement pin may potentially lead to an unintended discharge of your rifle.
You should stop using the rifle immediately and return it to Smith & Wesson so that the appropriate corrective action can be taken.

Smith & Wesson M&P45
M&P 45 Pistol - Sales in Massachusetts
DATE: March 24, 2008
On March 11, 2008, Smith & Wesson published a list of firearms recently approved for
sale in the state of Massachusetts. Included in this list were the following M&P 45
Pistols - SKU #'s 109006, 109106, 109156 and 109556.
It has come to our attention that the M&P 45 Pistol may not meet the requirements of the
Massachusetts Attorney General Regulations (940CMR 16.00). We are therefore asking
all dealers to stop all sales of this pistol in the state of Massachusetts.
At this time we are requesting that dealer's return all M&P45 Pistols back to the
distributor they purchased them from. Please contact any consumer that purchased a
M&P45 and ask them to return it to you for a full refund. If you have any questions please
contact your Smith & Wesson distributor.
Smith & Wesson is continuing the engineering review to ensure that the M&P 45 is
compliant for sale in the state of Massachusetts. It is our intention to get the pistol
approved for sale in the state of Massachusetts.
I apologize for the inconvenience that this has caused.

ATF 4473 Form Error
Some 4473 Forms are Invalid Due to Printing Error
DATE: March, 2008
A printer's error has invalidated some recently issued ATF Form 4473 Firearms Transaction Record Part I Over-The Counter forms, which are used for the retail sale of a firearm by a licensed dealer.
On some of these misprinted forms, the form number, date of release (month/year), and the language "previous editions are obsolete", were inadvertently left off by the printer. ATF advises that though its Distribution Center may have issued such misprinted forms to licensees, the documents cannot be used.
To order replacement forms, contact the ATF Distribution Center either by telephone at 703-455-7801; through its web site at www.atf.gov or by submitting ATF Form 1370.2 Requisition For Firearms/Explosives Forms.
